Back in the good old days or bad, depending on your perspective, the lunch on expenses was an unofficial bonus for us junior guys in the industry.
One day our superior boss asked my colleague to arrange lunch to entertain major clients at one of my favourite restaurants. Everything went well until my boss asked him to get the bill.
It was then that Manuel the head waiter in plain hearing of us all said " of course sir, would that be a blank receipt as usual?"
That was surely the moment for a Basil Fawlty slap around the head.

    Nice one Steve. You beat me to the punch here as I was about to tell a very similar true story about my old mate Chris, but I'll tell it anyway as it underscores the veracity of the practice back then. :-)
